As a signatory of Annex 1 – Registration slip, representative of the juvenile whose name I have written in Annex 1 – Registration slip, I hereby confirm that I agree that he/she participate in the International Online Contest of the Festival 4 Arts.

I agree that the Work (see Annex 2) of the juvenile, sent for the contest to be used by the organizers only for non-commercial purposes and only for the purpose of promoting the event and the image of the Festival 4 Arts, Câmpina.

I mention that I have read and complied with the articles in Annex 2 – PUBLICATION AGREEMENT and have taken note of all the conditions described in Annex 1 and in the Rules & Regulations of the competition.       

The Date and signature of the Author of the Paper in Annex 1 are valid
